Researchers advocate for the integration of mental health screenings and professionals into neuromuscular care teams to address the prevalent yet often unmet psychosocial needs of these patients and their families. The study, titled “Psychological symptoms in individuals with Spinal Muscular Atrophy (SMA) and their caregivers – results from a nation-wide study in Germany,” was published in the Orphanet Journal of Rare Diseases.

SMA is primarily caused by mutations in the SMN1 gene, leading to motor neuron loss and symptoms such as muscle weakness. The mental health challenges faced by SMA patients, particularly issues like depression and anxiety, are more pronounced in children compared to external behavioral problems such as hyperactivity. However, there is a lack of comprehensive data regarding adult SMA patients and their caregivers, especially concerning the correlation between mental health symptoms and specific SMA types. Furthermore, research into the effects of contemporary SMA treatments on psychological wellbeing is limited.

The study evaluated the psychological wellbeing of 103 SMA patients from the national German SMA registry, alongside 67 caregivers. Among the patients, 82 were adults with a median age of 42.5 years, while 21 were children or adolescents with a median age of 9.7 years. Most participants (57%) had SMA type 3, followed by type 2 (36%) and type 1 (7%). Approximately two-thirds of the patients could sit up, and 26% were able to walk independently. Treatment-wise, 55% were receiving Spinraza (nusinersen), while 22% were either on Evrysdi (risdiplam) or untreated.

Among children with SMA, parental reports indicated that 9.5% exhibited clinically relevant mental health symptoms, with 19% showing emotional problems and 9.5% facing peer interaction issues. Emotional distress was particularly noted in children with SMA type 3, while peer issues were reported in 50% of children with SMA type 1. No clinically significant symptoms were observed in the seven children who self-assessed their mental health.

For adults with SMA, 13.4% reported clinically significant psychological symptoms, largely encompassing anxiety (13.4%), depression (11.1%), and somatization, where emotional distress appears as physical symptoms (14.6%). Caregivers also reported mental health challenges, with 14.9% experiencing symptoms, including anxiety and somatization. Symptoms were particularly prevalent among caregivers of patients with SMA type 1.

The study found that pharmacological treatments do not significantly alleviate psychological symptoms among SMA individuals or their caregivers. The researchers noted that the extreme physical limitations faced by children with SMA type 1, such as the need for respiratory support, may lead to social challenges and affect interactions with peers. Despite advancements in pharmacological treatments leading to improved outcomes and extended survival, the necessity for enhancing social participation remains critical.

Interestingly, adults who were not receiving treatments like Spinraza or Evrysdi demonstrated higher rates of somatization and anxiety compared to those who were treated. The association between the mental health of SMA patients and their caregivers indicates a potential interconnectedness of psychological wellbeing within families.

The findings underscore the importance of integrated psychosocial care tailored for individuals with SMA and their caregivers. Routine psychological screenings are advisable to identify emerging mental health issues early and ensure access to mental health services for both patients and their families.
